Medical Transcription Industry in the Philippines
Medical ‘transcriptionists’ are earning big bucks transcribing medical records at home.
Medical transcription is the job of typing voice recordings made by physicians or other healthcare professionals into medical reports and correspondence. The people who make such transcriptions are called medical “transcriptionists”: transcribers who produce discharge summaries, physical examinations reports, patients’ history reports, operating room reports, consultation notes, diagnostic imaging studies, autopsy reports, and referral letters at home or in the office. They listen to recordings on a special headset and use a foot pedal to pause the recording when desired. As they listen, they type the text using word processing software, organize the material into a set format, and then return transcribed documents to the dictator for review and signature or correction.
They use their understanding of medical terminology, anatomy and physiology, diagnostic procedures, and treatment to create accurate reports, says Lynn Shniper in her article, Medical transcritionists: Making medical histories, in the Fall 2001 edition of the Occupational Outlook Quarterly. They translate medical abbreviations and jargon into their expanded forms and check their spellings and meanings by consulting standard medical reference materials. The more experienced of them spot inconsistencies or mistakes, such as misspoken prescriptions, in a medical report and verify the correct information with the dictator. Their ability to understand and correctly transcribe patient assessments and treatments reduces the chance that patients will receive ineffective or harmful care.
In the United States, most medical transcribers receive special training. Employers usually prefer to hire those who have postsecondary education in medical transcription, which is available through many vocational schools, community colleges, and distance-learning programs. Demand for transcribers’ services is so high that U.S. hospitals, physicians, business support services, and medical transcription companies regularly subcontract transcribing jobs at home and abroad.
In its Occupational Outlook Handbook for 2004-2005, the United States Bureau of Labor Statistics says “Employment of medical transcriptionists is projected to grow faster than the average for all occupations through 2012.” The United States’ growing and aging population requires medical documentation that can be shared by providers, third-party payers, regulators and consumers, but the increasing volume of transcription work being contracted out overseas will not reduce the need for well-trained medical transcribers at home.
Outside the United States, India is the recognized leader in providing medical transcription services to U.S. institutions. But the Philippines is fast catching up as the country of choice for offshore transcriptions as a result of its large pool of healthcare professionals. “India is the leading provider because they offer cheap labor, but the Philippines has the competitive edge as we can provide quality service at reasonable prices,” says Oliver Juanir, co-owner of Advatech Medical Transcription Specialists. “We started just several months ago, but clients continue to pour in.”
Juanir, an entrepreneur on the lookout for business opportunities, was surfing the Web when he stumbled on medical transcription services and then did some research. Next, he pitched his business plan to several investors and got one to put up P2 million to buy equipment (top-of-the-line personal computers, file transfer protocol or FTP servers, medical dictionaries, and computer software), plug into the Internet, and rent office space. He incorporated Advatech in August and spent two months recruiting staff. “I made it a point to recruit only those who had experience,” he says. He e-mailed medical transcription companies in the U.S., and once Advatech passed some tests for minimum output, clients, all medical transcription companies, started calling. It started with four transcribers to work for three doctors initially, but in four months it hired eight more to serve 20 doctors in four U.S. hospitals. “We’re still hiring as projects continue to pour in,” says Juanir.
Advatech’s transcribers start work by downloading audio files from the FTP server at 7 a.m. They then transcribe the files before forwarding the finished work to the editors for checking, observing a 12-hour deadline for each project. Medical transcription companies are paid five to eight U.S. cents per line of about 65 characters transcribed, but Advatech charges only four to seven U.S. cents to get more clients. It sets a 500-line daily quota for its transcribers-those exceeding their quota receive a commission-and gets paid monthly for its services.
Many medical transcribers get to like what they do. Michelle Diamante has been transcribing medical reports at home for three years, and she likes it because “You’re not forced to work an eight-hour job. You work at your own pace and earn in dollars.” She uses her speed, accuracy, and knowledge of medical terms to thrive in her calling, something she’d stumbled on as a fresh graduate of physical therapy. “It was hard to get a job in hospitals then, so I applied with a medical transcription company,” she says. She trained for two months after she got the job, and then quit after a year and a half to transcribe at home using referrals from her officemates to secure projects. All she needed to get started were a PC, an Internet connection, and a foot pedal to control dictation speed, and when she transcribed her first test files and passed, she resumed work as a medical transcriber in the comfort of home.
The outsourcing company simply provided her with a user name and password so she could download audio files from an encrypted website. “Since I operate in the same time zone as that of the US, I usually work from 8 p.m. onwards depending on the amount of work I would like to take in,” she says. Today, Diamante works as an editor of medical files transcribed by others. Medical transcribers get paid P20,000 to P50,000 a month depending on the amount of work they do, she says, adding the outsourcing company typically credits the money they’ve earned to their bank accounts. “We’re usually given a 24-hour deadline. The more transcribed work you edit, the more you get paid.”
Myla Jane Reyes, another home-based transcriber, and a physical therapist like Diamante, says there’s a big demand for medical transcription services in the Philippines. “The business is good and work comes even on weekends and holidays,” she says. Reyes trained for six months before she was allowed to do transcription work by one of the pioneer medical transcription firms in the country. She stayed with that company for two years until it closed, and then she joined another company before she decided to do extra transcription work at home on the advice of a friend, who had come across an Internet ad looking for home-based medical transcribers.
“In 2002 I applied for the job and passed their online exam, so I pursued it,” she says. For a whole year, Reyes transcribed in the office by day and at home at night and on weekends. “I had a hard time choosing between office work and home-based transcription as it was not popular back then,” she says, but after a year she decided to quit her regular job and concentrated on home-based transcribing and editing. “The risk I took was worth it,” she says. “I never had regrets choosing home transcription.”
Assessing Privacy Risk in Outsourcing
Healthcare providers can outsource transcription, but they can’t outsource their obligation to safeguard privacy. Here’s how to minimize risk.
Healthcare providers are faced with multiple pressures, many of them financial. The need for management to meet financial constraints often translates into a desire to contract with vendors at the lowest possible immediate cost, sometimes without thought as to the nonprice issues in a contract.
It is important to linger over the legal issues that may be associated with vendor contracts, especially with vendors that may subcontract portions of their tasks. The October 2003 incident in which a Pakistani subcontractor, in a dispute with a medical transcription company, threatened to release patient information on the Web provided a dramatic reminder of this. Provider and vendor liability has become even more important in light of HIPAA privacy and security regulations, which place an obligation on covered healthcare providers to ensure that their vendors safeguard confidentiality.
This article discusses specifically the considerations that should be given by healthcare providers when choosing a medical transcription vendor, and it offers tips to protect providers when entering into a medical transcription contract.
Transcription OutsourcingMedical transcription is a vital part of healthcare operations. Accurate, timely transcription of daily notes such as operating room reports, discharge summaries, and radiology reports is essential for communication among healthcare providers. Transcription further factors into accurate coding and billing of services, satisfaction of regulatory requirements, and defense of medical malpractice suits. In recent years it has become important in the development of electronic health records.
Transcription is also a frequent candidate for outsourcing. Healthcare providers often contract out their transcription as a cost-saving measure (though cost savings may not be possible in all circumstances). Providers also choose outsourcing to meet staffing challenges posed by local labor markets, the need for specialized training, and their own staffing limitations.
Whatever the motivation for outsourcing, transmission of confidential medical information outside of facility walls places an obligation on the provider to ensure that the vendor protects the confidentiality of the information. HIPAA requirements make this issue more critical than ever before.
The case in which the Pakistani subcontractor threatened to release patient information illustrates how important it is for providers to choose vendors carefully to complete due diligence with regard to chains of subcontracting and to protect themselves contractually from liability for acts of the vendor or the vendor’s subcontractors.
The above story started in fall 2003 when the University of California at San Francisco Medical Center (UCSF) forwarded a portion of its transcription work to Transcription Stat, a company it had used for two decades. Transcription Stat employs 15 subcontractors throughout the US to handle the large volume of files it receives daily from UCSF. One of those subcontractors, a woman in Florida, further subcontracted the work to a man in Texas named Tom Spires.
Allegedly unbeknownst to the other parties, Spires also employed subcontractors, one of whom was a Pakistani woman, Lubna Baloch. On October 7, 2003, UCSF received an e-mail from Baloch stating that Spires owed her money and would not respond to her. Baloch demanded that UCSF require Spires to pay her. If not paid, Baloch wrote, she would “expose all the voice files and patient records of UCSF . . . on the Internet.”1 To show that she was serious, Baloch attached dictation reports from UCSF physicians regarding two patients.
One of the contracted parties involved ultimately paid Baloch, who then retracted her threat. Although the incident ended without a breach of patient privacy, the situation dramatically illustrated the risks for parties involved at all points in the chain. How can a provider best protect itself from a situation such as this?
What HIPAA Requires The privacy regulations of the federal HIPAA law, effective April 14, 2003, were intended to ensure the privacy and confidentiality of personal health information. HIPAA’s privacy rules apply only to healthcare providers, payers, and clearinghouses that are covered entities. Because the law does not directly apply to other parties that may handle medical information (e.g., transcription companies and other vendors), HIPAA attempts to stretch its coverage by requiring healthcare providers to take certain actions when entrusting medical information to vendors.
HIPAA requires that if a provider releases medical information to another person or entity to perform a function on the provider’s behalf (the provider’s “business associate”), the provider must enter into an agreement obliging the business associate to maintain the confidentiality of the medical information.
HIPAA’s privacy regulations require that business associate contracts contain several specific provisions. The contract must specify the permitted uses and disclosures of information by the business associate. The contract also must require the business associate not to use or further disclose the information except as permitted by the contract and to use appropriate safeguards to prevent use or disclosure of the information other than as allowed by the contract. The contract must “ensure that any agents, including a subcontractor,” to whom the business associate provides medical information agree to the “same restrictions and conditions that apply to the business associate with respect to such information.” The contract must authorize termination if the business associate violates a material term of the contract.
When the HIPAA security rule goes into effect, medical transcription vendors will be covered as business associates under this rule also, because the security rule applies to electronic medical information, both in storage and in transmission. The rule requires that when providers contract with business associates who “create, receive, maintain or transmit electronic protected health information,” the contract specify appropriate use of the information.
Additional security provisions may need to be added to business associate contracts with transcription vendors and other business associates that receive or transmit electronic information. For example, the security regulations require that any business associate shall “(i) implement administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity and availability of electronic protected health information that it creates, receives, maintains or transmits on behalf of the covered entity, as required by the security standards, (ii) ensure that any agent, including subcontractors to whom the business associate provides such information, agrees to implement reasonable and appropriate safeguards to protect it, and (iii) report to the covered entity any security incident relating” to the electronic information that the vendor maintains for the provider.
What Providers Should DetermineProviders should therefore determine how a medical transcription vendor maintains the privacy of medical information and how the vendor safeguards the security of electronic information. Providers should get answers to the following questions:
•Does the vendor have security safeguards for information that is maintained at the vendor’s data headquarters (e.g., is each employee required to have a secure password)? Are voice files and data files maintained in a secure data safe?•Is there an audit trail that shows who has accessed data in the vendor’s archive system?•How long are data files maintained, and are they destroyed after that period? Are voice files automatically deleted within a reasonable time after being transcribed? The provider may wish to specify that data is maintained only until the provider ensures that the transcribed report is in the medical record (e.g., no more than one year).•Is electronic medical information transmitted from the vendor to the provider in a secure fashion—for example, through encryption?•How does the vendor regulate provider access to the provider’s directory on the vendor server? For example, is it safest to allow only one person authorized by the provider (e.g., the director of medical records) to draw data down from the transcription vendor’s computer system?•If the vendor employs home-based transcriptionists, does the vendor have policies to safeguard workstations? Are home-based transcriptionists allowed to store provider files on their home computers? Minimizing Risk Providers can consider a number of options when entering into a contract for medical transcription services. These options can clarify the mutual understanding of the contractual relationship, minimize the chance of a HIPAA or confidentiality violation, and ensure that the provider will be able to take appropriate action should a vendor fail to meet required standards.
1.Ensure that the contract with the medical transcription company obligates the vendor to not only maintain confidentiality itself, but to require any person or entity to whom the vendor sends information to maintain confidentiality and security of information. The vendor should require that the person or entity comply with all of the obligations of the vendor under the vendor’s business associate agreement with the provider.In the UCSF case, the transcription vendor, the first subcontractor, and second subcontractor were simply intermediaries. That realization makes a provider wonder how many transcription vendors operate by simply contracting with another party whose cost is low enough to allow the first vendor to skim off a profit, and so on down the line until the only person left to do the work is someone whose wages are below those livable in the US. It is difficult to ascertain how HIPAA’s privacy or security requirements can be satisfied if a transcription company is simply a pass-through: can anyone identify who has the data, how it is stored, or where it is? Nevertheless, the HIPAA rule assists providers by making clear that vendors are responsible for the privacy and security of the data given to them.
The obligations of both provider and business associate become further complicated if some of the parties receiving information are not located inside the United States. Entities not domiciled in the US may not be subject to, or even aware of, US laws.
2.Require indemnification from the vendor for any breach of the contract (including confidentiality), not only by the vendor but by any of the vendor’s subcontractors or entities to which the subcontractor may send information.3.Consider placing restrictions on the subcontractors that a vendor may use; for instance, explicitly requiring that any subcontractor receiving medical information from the vendor be physically located in the United States and that, in addition, the vendor’s subcontractors be explicitly prohibited from sending any of the provider’s information outside the US. Although this will not obviate the problems that can occur if a subcontractor isn’t paid and threatens release of information, for example, it will ensure that any such subcontractor is subject to US law and may be less likely to make such threats. Also, providers can consider asking vendors outright for a full disclosure of subcontracts.4.Consider using a medical transcription company that does not subcontract any work at all. Some vendors are staffed with full-time employees who receive benefits, and some offer incentives to staff for meeting high performance standards. Some vendors assign transcriptionists responsibility for a single client, fostering familiarity with the client’s transcription needs. Such vendors may charge a premium for that level of service, but providers may want to balance that cost with the potential benefits of a stable, dedicated staff.5.Consider whether the transcription company is making investments to obtain and retain the provider as a customer. Is it purchasing computers and equipment for use on the account, or is it simply subcontracting all work and keeping a percentage of the fee ? The answer gives the provider a clue as to how much the vendor values the relationship and how carefully other contract terms should be reviewed.6.Ensure protection by including specific performance standards in the contract. These should include turnaround time, error rate, and template consistency (so that the documents follow a standard format). Not only are written performance standards important to give the provider the ability to terminate the contract if standards are not met, they also can help identify hidden costs. Although one vendor may appear to be less costly than another, does a high error rate require the provider to allocate time from other employees to review, edit, and correct the vendor’s work? 7.Weigh the cost of training staff regarding confidentiality. HIPAA requires that all persons with access to personal health information receive training on the confidentiality requirements of the law. In addition, laws in some states require additional staff training for specific situations, such as New York’s AIDS confidentiality law. It can be easier for the provider to place the burden of training on the transcription company. 8.Ensure that the contract contains the protection terms standard in any contract: (1) the ability (of both parties) to terminate the contract for cause (e.g., failure to comply with the terms of the contract) and not for cause (if the vendor has made a substantial capital investment in the provider’s account, this clause may only be agreed upon if the provider takes some responsibility for the investment); (2) an appropriate length of time for the contract; (3) inability of the vendor to assign the contract without the provider’s permission; and (4) a requirement that any claim be brought in the state in which the provider is located. In the wake of the UCSF case, a California state senator discussed introducing legislation that would prohibit provider and payer organizations in California from sending confidential medical information outside of the United States for transcription or other outsourced data processing activities.
Such a law would ensure that all vendors and their subcontractors are subject to US laws, but it would not alter a provider’s core responsibilities when transmitting confidential records to a vendor. Meeting responsibilities and minimizing risk will still require asking the right questions, weighing the options, and establishing an appropriate business associate agreement.

They generally listen to recordings on a headset, using a foot pedal to pause the recording when necessary, and key the text into a personal computer or word processor, editing as necessary for grammar and clarity. The documents they produce include discharge summaries, medical history and physical examination reports, operative reports, consultation reports, autopsy reports, diagnostic imaging studies, progress notes, and referral letters. Medical transcriptionists return transcribed documents to the physicians or other health care professionals who dictated them for review and signature or correction. These documents eventually become part of patients’ permanent files.
To understand and accurately transcribe dictated reports, medical transcriptionists must understand medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessments. They also must be able to translate medical jargon and abbreviations into their expanded forms. To help identify terms appropriately, transcriptionists refer to standard medical reference materials—both printed and electronic; some of these are available over the Internet. Medical transcriptionists must comply with specific standards that apply to the style of medical records and to the legal and ethical requirements for keeping patient information confidential.
Experienced transcriptionists spot mistakes or inconsistencies in a medical report and check to correct the information. Their ability to understand and correctly transcribe patient assessments and treatments reduces the chance of patients receiving ineffective or even harmful treatments and ensures high-quality patient care.
Currently, most health care providers transmit dictation to medical transcriptionists using either digital or analog dictating equipment. The Internet has grown to be a popular mode for transmitting documentation. Many transcriptionists receive dictation over the Internet and are able to quickly return transcribed documents to clients for approval. Another increasingly popular method uses speech recognition technology, which electronically translates sound into text and creates drafts of reports. Transcriptionists then format the reports; edit them for mistakes in translation, punctuation, or grammar; and check for consistency and any wording that doesn’t make sense medically. Transcriptionists working in specialties, such as radiology or pathology, with standardized terminology are more likely to use speech recognition technology. However, speech recognition technology will become more widespread in all specialties as the technology becomes more sophisticated, that is, better able to recognize and more accurately transcribe diverse modes of speech.
Medical transcriptionists who work in physicians’ offices may have other office duties, such as receiving patients, scheduling appointments, answering the telephone, and handling incoming and outgoing mail. Medical secretaries, discussed in the statement on secretaries and administrative assistants elsewhere in the Handbook, also may transcribe as part of their jobs.
Work environment. The majority of these workers are employed in comfortable settings, such as hospitals, physicians’ offices, transcription service offices, clinics, laboratories, medical libraries, government medical facilities, or their own homes. Many medical transcriptionists telecommute from home-based offices.
Workers usually sit in the same position for long periods. They can suffer wrist, back, neck, or eye problems due to strain and risk repetitive motion injuries such as carpal tunnel syndrome. The constant pressure to be accurate and productive also can be stressful.
Many medical transcriptionists work a standard 40-hour week. Self-employed medical transcriptionists are more likely to work irregular hours—including part time, evenings, weekends, or on call at any time.
Training, Other Qualifications, and Advancement
Postsecondary training in medical transcription is preferred by employers; writing and computer skills also are important.
Education and training. Employers prefer to hire transcriptionists who have completed postsecondary training in medical transcription offered by many vocational schools, community colleges, and distance-learning programs.
Completion of a 2-year associate degree or 1-year certificate program—including coursework in anatomy, medical terminology, legal issues relating to health care documentation, and English grammar and punctuation—is highly recommended, but not always required. Many of these programs include supervised on-the-job experience. Some transcriptionists, especially those already familiar with medical terminology from previous experience as a nurse or medical secretary, become proficient through refresher courses and training.
Formal accreditation is not required for medical transcription programs. However, the Approval Committee for Certificate Programs (AACP)—established by the Association for Healthcare Documentation Integrity (AHDI) and the American Health Information Management Association—offers voluntary accreditation for medical transcription programs. Although voluntary, completion of an ACCP approved program may be required for transcriptionists seeking certification.
Certification and other qualifications. The AHDI awards two voluntary designations, the Registered Medical Transcriptionist (RMT) and the Certified Medical Transcriptionist (CMT). Medical transcriptionists who are recent graduates of medical transcription educational programs, or have fewer than 2 years experience in acute care, may become a registered RMT. The RMT credential is awarded upon successfully passing the AHDI level 1 registered medical transcription exam. The CMT designation requires at least 2 years of acute care experience working in multiple specialty surgery areas using different format, report, and dictation types. Candidates also must earn a passing score on a certification examination. Because medicine is constantly evolving, medical transcriptionists are encouraged to update their skills regularly. RMTs and CMTs must earn continuing education credits every 3 years to be recertified. As in many other fields, certification is recognized as a sign of competence.
Graduates of an ACCP approved program who earn the RMT credential are eligible to participate in the Registered Apprenticeship Program sponsored by the Medical Transcription Industry Association through the U.S. Department of Labor. The Registered Apprenticeship program offers structured on-the-job learning and related technical instruction for qualified medical transcriptionists entering the profession.
In addition to understanding medical terminology, transcriptionists must have good English grammar and punctuation skills and proficiency with personal computers and word processing software. Normal hearing acuity and good listening skills also are necessary. Employers usually require applicants to take pre-employment tests.
Advancement. With experience, medical transcriptionists can advance to supervisory positions, home-based work, editing, consulting, or teaching. Some become owners of medical transcription businesses. With additional education or training, some become medical records and health information technicians, medical coders, or medical records and health information administrators.
Employment
Medical transcriptionists held about 98,000 jobs in 2006. About 41 percent worked in hospitals and another 29 percent worked in offices of physicians. Others worked for business support services; medical and diagnostic laboratories; outpatient care centers; and offices of physical, occupational, and speech therapists, and audiologists.
Job Outlook
Employment of medical transcriptionists is projected to grow faster than the average; job opportunities should be good, especially for those who are certified.
Employment change. Employment of medical transcriptionists is projected to grow 14 percent from 2006 to 2016, faster than the average for all occupations. Demand for medical transcription services will be spurred by a growing and aging population. Older age groups receive proportionately greater numbers of medical tests, treatments, and procedures that require documentation. A high level of demand for transcription services also will be sustained by the continued need for electronic documentation that can be shared easily among providers, third-party payers, regulators, consumers, and health information systems. Growing numbers of medical transcriptionists will be needed to amend patients’ records, edit documents from speech recognition systems, and identify discrepancies in medical reports.
Contracting out transcription work overseas and advancements in speech recognition technology are not expected to significantly reduce the need for well-trained medical transcriptionists. Outsourcing transcription work abroad—to countries such as India, Pakistan, Philippines, and the Caribbean—has grown more popular as transmitting confidential health information over the Internet has become more secure; however, the demand for overseas transcription services is expected only to supplement the demand for well-trained domestic medical transcriptionists. In addition, reports transcribed by overseas medical transcription services usually require editing for accuracy by domestic medical transcriptionists before they meet U.S. quality standards.
Speech-recognition technology allows physicians and other health professionals to dictate medical reports to a computer that immediately creates an electronic document. In spite of the advances in this technology, the software has been slow to grasp and analyze the human voice and the English language, and the medical vernacular with all its diversity. As a result, there will continue to be a need for skilled medical transcriptionists to identify and appropriately edit the inevitable errors created by speech recognition systems, and to create a final document.
Job prospects. Job opportunities will be good, especially for those who are certified. Hospitals will continue to employ a large percentage of medical transcriptionists, but job growth there will not be as fast as in other industries. An increasing demand for standardized records should result in rapid employment growth in physicians’ offices, especially in large group practices.

Nature of the Work:Medical transcriptionists listen to dictated recordings made by physicians and other healthcare professionals and transcribe them into medical reports, correspondence, and other administrative material. They generally listen to recordings on a headset, using a foot pedal to pause the recording when necessary, and key the text into a personal computer or word processor, editing as necessary for grammar and clarity. The documents they produce include discharge summaries, history and physical examination reports, operative reports, consultation reports, autopsy reports, diagnostic imaging studies, progress notes, and referral letters. Medical transcriptionists return transcribed documents to the physicians or other healthcare professionals who dictated them for review and signature, or correction. These documents eventually become part of patients permanent files. To understand and accurately transcribe dictated reports into a format that is clear and comprehensible for the reader, medical transcriptionists must understand medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessments. They also must be able to translate medical jargon and abbreviations into their expanded forms. To help identify terms appropriately, transcriptionists refer to standard medical reference materials both printed and electronic; some of these are available over the Internet. Medical transcriptionists must comply with specific standards that apply to the style of medical records, in addition to the legal and ethical requirements involved with keeping patient information confidential.
Experienced transcriptionists spot mistakes or inconsistencies in a medical report and check to correct the information. Their ability to understand and correctly transcribe patient assessments and treatments reduces the chance of patients receiving ineffective or even harmful treatments and ensures high quality patient care.
Currently, most healthcare providers transmit dictation to medical transcriptionists using either digital or analog dictating equipment. The Internet has grown to be a popular mode for transmitting documentation. Many transcriptionists receive dictation over the Internet and are able to quickly return transcribed documents to clients for approval. Another emerging trend is the implementation of speech recognition technology, which electronically translates sound into text and creates drafts of reports. Reports are then formatted; edited for mistakes in translation, punctuation, or grammar; and checked for consistency and possible medical errors. Transcriptionists working in areas with standardized terminology, such as radiology or pathology, are more likely to encounter speech recognition technology. However, use of speech recognition technology will become more widespread as the technology becomes more sophisticated.
Medical transcriptionists who work in physicians offices and clinics may have other office duties, such as receiving patients, scheduling appointments, answering the telephone, and handling incoming and outgoing mail. Medical secretaries, discussed in the statement on secretaries and administrative assistants elsewhere in the Handbook, may also transcribe as part of their jobs. Court reporters, also discussed elsewhere in the Handbook, have similar duties, but with a different focus. They take verbatim reports of speeches, conversations, legal proceedings, meetings, and other events when written accounts of spoken words are necessary for correspondence, records, or legal proof.
Working Conditions:The majority of these workers are employed in comfortable settings, such as hospitals, physicians offices, transcription service offices, clinics, laboratories, medical libraries, government medical facilities, or at home. Many medical transcriptionists telecommute from home-based offices as employees or subcontractors for hospitals and transcription services or as self-employed, independent contractors. Work in this occupation presents hazards from sitting in the same position for long periods, and workers can suffer wrist, back, neck, or eye problems due to strain and risk repetitive motion injuries such as carpal tunnel syndrome. The pressure to be accurate and productive also can be stressful.
Many medical transcriptionists work a standard 40-hour week. Self-employed medical transcriptionists are more likely to work irregular hours including part time, evenings, weekends, or on-call at any time.
Employment:Medical transcriptionists held about 101,000 jobs in 2002. About 4 out of 10 worked in hospitals and another 3 out of 10 worked in offices of physicians. Others worked for business support services, offices of other health practitioners, medical and diagnostic laboratories, outpatient care centers, and home healthcare services.
Training, Other Qualifications,and Advancement:Employers prefer to hire transcriptionists who have completed postsecondary training in medical transcription, offered by many vocational schools, community colleges, and distance-learning programs. Completion of a 2-year associate degree or 1-year certificate program including coursework in anatomy, medical terminology, legal issues relating to healthcare documentation, and English grammar and punctuation is highly recommended, but not always required. Many of these programs include supervised on-the-job experience. Some transcriptionists, especially those already familiar with medical terminology due to previous experience as a nurse or medical secretary, become proficient through on-the-job training. The American Association for Medical Transcription (AAMT) awards the voluntary designation, Certified Medical Transcriptionist (CMT), to those who earn passing scores on written and practical examinations. As in many other fields, certification is recognized as a sign of competence. Because medical terminology is constantly evolving, medical transcriptionists are encouraged to regularly update their skills. Every 3 years, CMTs must earn continuing education credits to be recertified.
In addition to understanding medical terminology, transcriptionists must have good English grammar and punctuation skills, as well as proficiency with personal computers and word processing software. Normal hearing acuity and good listening skills also are necessary. Employers often require applicants to take pre-employment tests.
With experience, medical transcriptionists can advance to supervisory positions, home-based work, editing, consulting, or teaching. With additional education or training, some become medical records and health information technicians, medical coders, or medical records and health information administrators.
Job Outlook:Job opportunities will be good. Employment of medical transcriptionists is projected to grow faster than the average for all occupations through 2012. Demand for medical transcription services will be spurred by a growing and aging population. Older age groups receive proportionately greater numbers of medical tests, treatments, and procedures that require documentation. A high level of demand for transcription services also will be sustained by the continued need for electronic documentation that can be easily shared among providers, third-party payers, regulators, and consumers. Growing numbers of medical transcriptionists will be needed to amend patients records, edit for grammar, and identify discrepancies in medical records. Contracting out transcription work overseas and advancements in speech recognition technology are not expected to significantly reduce the need for well-trained medical transcriptionists domestically. Contracting out transcription work abroad to countries such as India has grown more popular as transmitting confidential health information over the Internet has become more secure; however, the demand for overseas transcription services is expected to supplement the demand for well-trained domestic medical transcriptionists. Speech-recognition technology allows physicians and other health professionals to dictate medical reports to a computer that immediately creates an electronic document. In spite of the advances in this technology, it has been difficult for the software to grasp and analyze the human voice and the English language with all its diversity. As a result, there will continue to be a need for skilled medical transcriptionists to identify and appropriately edit the inevitable errors created by speech recognition systems, and create a final document.
Hospitals will continue to employ a large percentage of medical transcriptionists, but job growth there will not be as fast as in other industries. Increasing demand for standardized records should result in rapid employment growth in offices of physicians or other health practitioners, especially in large group practices.

Job Description: MD-IT is seeking an experienced Medical Transcription/Operations Manager to work in-house in its Eugene, OR office. This is NOT an at-home position. JOB SUMMARY: Direct managerial responsibility for all off-site medical transcriptionists across all time zones for the Oregon office. This responsibility includes the monitoring of transcription activity relative to staffing and training needs as well as the identification of performance and client servicing issues, working with Customer Support, Implementations, Finance, and QA, as applicable, to resolve such issues. The Trans/Ops Manager shall be responsible for scheduling of transcriptionists hours to ensure that client turnaround times are met, providing regular feedback to all transcriptionists (both positive and negative), and conducting the performance reviews for all transcriptionists. The Trans/Ops Manager shall make recommendations for work placement, cross training and staffing based on MD-IT Oregon's business needs. The Trans/Ops Manager shall also address transcription specific client servicing issues and work with office staff and Managing Director to implement strategies for resolution. Additional duties include compiling relevant business statistics for both the Managing Director and transcriptionists, overseeing and administering the office's QA and training program as well as assisting with the effective implementation of all new enhancements or procedural changes. The Trans/Ops Manager will work with Central Recruiting and the Managing Director to determine hiring needs, conduct final interviews, make hiring recommendations and set up training schedules for all new medical transcriptionists. SKILLS AND EDUCATIONAL REQUIREMENTS: Ability to communicate effectively, both verbally and in writing. Excellent organizational and managerial skills. Strong people and analytical skills and an ability to prioritize tasks and work independently. Excellent grammar, medical terminology and computer skills. Thorough knowledge of AHDI Book of Style. Previous transcription background in a variety of medical specialties (4 years). Previous transcription operations management experience (2 years). CMT credential from AHDI preferred.
